Terms of Service for Constant Care Physiotherapy

Welcome to Constant Care Physiotherapy.
By accessing or using our services, you agree to comply with and be bound by these Terms of Service. If you do not agree to these terms, please do not use our services.
1. Introduction
Constant Care Physiotherapy is a physiotherapy clinic located in Ontario, Canada. These Terms of Service govern your use of the services we provide, including in-person treatments, telehealth consultations, and any related services we offer to our patients.
2. Services Provided
We offer a range of physiotherapy services, including but not limited to:
- Assessment and treatment of musculoskeletal injuries
- Manual therapy, exercise prescription, and rehabilitation programs
- Post-surgical rehabilitation
- Postural and movement assessments
- Education on injury prevention and management
All services provided are in accordance with the standards set by the College of Physiotherapists of Ontario and applicable provincial laws.
3. Appointment Scheduling and Cancellations
Appointments: Appointments for physiotherapy sessions can be made via email, or through our online booking system.
Cancellations: We require at least 24 hours' notice for appointment cancellations. If you cancel or reschedule within 24 hours of your appointment, you may be charged a cancellation fee of up to 50% of the service fee. Please visit our "Cancellation Policy" section for more details.
Late Arrivals: If you arrive late for your appointment, we may need to shorten the duration of the session to accommodate subsequent clients, and you may still be charged for the full session.
4. Payment and Billing
Fees: Fees for physiotherapy services will be disclosed before treatment begins and are payable upon completion of the session. The cost of services may vary based on the type and duration of the treatment provided.
Insurance: If applicable, we will provide you with a physiotherapy receipt to give to your private insurance provider or third-party payer. We are currently not setup for direct billing. It is your responsibility to verify your insurance coverage and to ensure that payments are made by the provider. You are responsible for any portion of the fee not covered by insurance.
Non-Payment: If you fail to pay for services rendered, we reserve the right to suspend treatment or take further action to recover outstanding amounts.
5. Patient Responsibilities
You are responsible for providing accurate health information to your physiotherapist, including any pre-existing medical conditions, injuries, medications, or treatments that may affect your care.
You must follow the treatment plan, exercises, and recommendations provided by your physiotherapist in order to achieve the best possible outcome.
It is your responsibility to ask questions and seek clarification if you do not understand any part of the treatment plan.
6. Privacy and Confidentiality
We are committed to protecting your privacy and handling your personal health information in compliance with the **Personal Health Information Protection Act (PHIPA)** and other applicable laws. Your medical records are confidential and will not be shared with third parties without your consent, except as required by law.
7. Consent to Treatment
By attending an appointment and receiving treatment at Constant Care Physiotherapy, you consent to the physiotherapy treatment provided by our staff. If you have any concerns or wish to withdraw consent, please notify your physiotherapist immediately.
8. Health and Safety
We prioritize the safety and well-being of our patients and staff. You agree to follow all safety protocols while at our clinic, including but not limited to:
- Wearing appropriate attire for physical activity.
- Following infection control measures (such as hand hygiene, mask-wearing if required).
- Reporting any changes in your health that may affect your treatment.
9. Limitation of Liability
While we make every effort to provide the highest standard of care, there are inherent risks in any medical treatment, including physiotherapy. We do not guarantee specific outcomes or results. You acknowledge that we are not liable for any adverse effects, injuries, or complications arising from treatment, except where such damages result from our negligence or failure to adhere to applicable standards of care.
10. Termination of Services
We reserve the right to terminate or suspend services at any time for reasons including, but not limited to, non-payment, failure to comply with treatment plans, or inappropriate behavior. You may also choose to discontinue treatment at any time, and we will provide a summary of your care upon request.
11. Intellectual Property
All content provided by Constant Care Physiotherapy, including but not limited to treatment materials, educational resources, logos, and website content, is owned by Constant Care Physiotherapy and protected by copyright laws. You may not reproduce, distribute, or use any content for commercial purposes without our prior written consent.
12. Changes to Terms of Service
We may update these Terms of Service from time to time. Any changes will be posted on our website, and the revised terms will be effective immediately upon posting. We encourage you to review these terms periodically.
13. Governing Law
These Terms of Service are governed by the laws of the Province of Ontario, Canada. Any disputes arising from these terms will be resolved in the courts of Ontario.
